SADI-S (Single Anastomosis Duodeno-ileal Bypass with Sleeve Gastrectomy) is a surgical technique for bariatric surgery whose purpose is to obtain adequate weight loss and the resolution of diseases associated with severe or morbid obesity.
This procedure, SADI-S as well as the classic duodenal crossing, bypass or sleeve gastrectomy, is a surgical technique used to lose weight. It also improves comorbidities such as diabetes, hyperlipidemia, sleep apnea and hypertension. The main advantage with the classic duodenal crossing is the completion of a single union with the intestine, which reduces the risks. The pylorus is preserved so it does not present complications such as dumping syndrome and the diet is better tolerated than in other surgeries.

The SADI-S type Bypass is a new operation to control obesity, which combines the advantages of sleeve gastrectomy with those of the classic Gastric Bypass or Duodenal Switch. It consists of performing a Gastric Tube Surgery, giving the stomach a sleeve shape, and after the exit of it, a derivation of the intestine is performed between the duodenum and ileum, which is what would be called the Bypass or Duodenal-ileal derivation.
With this we achieve a more physiological operation that provides a better quality of life for the patients. By preserving part of the stomach, as in sleeve gastrectomy, all the physiological functions of food entry, digestion and exit are preserved.
The "Gastric Bypass" operation is the most proven and well-known operation for the treatment of obesity. Besides, the "SADIS type Bypass" (Gastric Sleeve + Duodenal Bypass) is a new technique derived from the Gastric Sleeve, which achieves a more physiological Bypass, with fewer possible adverse effects, less need for vitamins and better quality of life.
Greater weight loss than with other bariatric surgeries such as sleeve gastrectomy or gastric bypass. Avoids the risk of reflux, flatulence or diarrhoea. Achieves significant control of diseases such as diabetes and the patient's quality of life improves compared to other bariatric surgeries.
This type of procedure is recommended for those patients with morbid obesity, with a body mass index higher than 55 and with diseases derived from obesity itself, such as type 2 diabetes, hypertension or high concentration of lipids in the blood, cholesterol, which are very severe and generate a serious risk to the patient.
It is currently being carried out on all 5 continents, although by selected teams, because it is a technically complex and very demanding operation for the surgeon and his team.
In Spain there are already more than 5000 patients operated and a follow-up of more than 10 years with excellent results in weight loss. This technique is ideal for people with a history of gastric sleeve that did not achieve complete weight loss or there is some weight regain.
The operation has excellent results with an approximate weight loss of 85% - 90% of excess weight, and because of its strength, it is indicated for patients with double morbid obesity or in patients with significant metabolic problems such as insulin-dependent diabetes, etc. Furthermore, it is the ideal technique for patients who, for whatever reason, have failed a previous sleeve gastrectomy operation.
